Application Process Minimize

The application process has six parts:

1.  Complete an Application for Admission, this form can be completed either by the Care Recipient or the Care Recipients Carer / Representative.

2.  ACAT Approval  -  A copy of this document needs to be provided to The Clem Jones Home

3.  Asset Assessment  -  An application for an asset assessment can be obtained through the Home or by contacting the Department of Health and Ageing.

Once this document has been completed it will need to be posted to either Centrelink or the Department of Veterans Affairs.  After their assessment has been completed a copy of the letter stating assets will need to be provided to the Home.

4.  Medical Assessment—this must be completed by the Care Recipients Doctor and provided to the Home.

5.  Certified copy of Enduring Power of Attorney

6.  Copy of Advanced Health Care Directive (if any)

Once the Application, Assets Assessment, Medical History Form and a copy of the ACAT has been provided your name will be placed on the Waiting List.  

Prior to your admission date you will be required to enter into a Residential Care Agreement which sets out the information pertaining to your residency at the Home.  You will also be required to provide a Certified Copy of Enduring Power of Attorney prior to admission, additionally if you have an Advanced Health Care Directive this also will need to be provided to the Home for your records.

It is advisable to regularly contact staff to confirm that accommodation is still required. Once a room becomes available, a time will be arranged for you to move in.
